Hello Foster Grandparent Site Supervisors,
Thank you for all you do as a site supervisor. Due to a high volume of questions pertaining to paperwork we have decided to inform you and answer your questions through the website. The following information goes over basic program information and the required mandatory paperwork that you are responsible for submitting.
The Foster Grandparent Program is a grant funded program and has grant driven requirements. Meaning it is essential to have regularly updated documents on file for ALL of our occupied sites.
The following documents that Site Supervisor are responsible for are:
- Memorandum of Understanding ( MOU's) (Signed every 3 years)
- Proof of a not-for_profit organization tax form (501(c)(3) (once upon placement)
- Volunteer Assignment Plan for each of the Foster Grandparents at each site (Due upon placement and annually Oct. thereafter)
- Annual Evaluation Forms (Due every January and May)
- In-Kind Form (Monthly);
-Sign In sign/ Sign out Sheet
We also ask for all of our Site Supervisors to NEVER have a Foster Grandparent left by them self, taking the children to the bathroom and/or changing diapers, or have a Foster Grandparent discipline a child. It is your responsibility if you are not in the classroom with the Foster Grandparent to inform and train the teacher who is supervising the Foster Grandparent of the responsibilities and the restrictions that they have.

This Sign in/Sign Out sheet is for sites to print and have for when Foster Grandparent come in the morning and when they leave for the day. The Foster Grandparent Program case managers will be coming around quarterly to collect them for auditing purposes. If your site already has a sign in/sign out sheet that is fine. We just want to make sure all sites have one.
• Sign In/ Sign Out SheetPaid Time off Policy (PTO) The Foster Grandparent program offers a Paid Time Off (PTO) benefit for the purpose of providing Foster Grandparent (FGP) volunteers with the opportunity to take time away from their volunteer assignment without loss of stipend. The PTO provided will replace all existing Pool Leave, Holiday leave, Bereavement and Administrative Leave allotted under prior policies. It is designed to encompass any absence from the work site, regardless of reason. EFFECTIVE DATE- This policy is effective July 1, 2010 upon receipt and supersedes any paid leave time policies previously in place.
